ABSTRACT

An 80-year-old woman was referred to our hospital with dyspnea on exertion. A close examination revealed angina pectoris, and coronary artery bypass grafting was planned. Preoperative unenhanced chest computed tomography showed a 4.2×3.2×5.0 cm-sized tumor in the anterior mediastinum. On magnetic resonance imaging, the lesion showed equal signal intensity on T1- and T2-weighted images. These radiological findings suggested thymoma. The operation was planned to be performed at the same time as coronary artery bypass grafting. Thymothymectomy was performed by median sternotomy. Histopathological examination showed spindle and oval epithelial tumor nodules separated by abundant lymphoid stroma. This appearance was consistent with micronodular thymoma with lymphoid stroma.
