ABSTRACT

A 76-year-old woman visited the hospital with left chest pain and malaise, and radiological findings showed left pleural effusion. Histological examination of the pleural biopsy showed fibrous thickening of the pleura and local IgG4-positive cells. On the other hand, she was diagnosed as having Sjögren's syndrome, based on xerostomia, xerophthalmia, and positive results for anti-Sjögren's syndrome (anti-SS-A/SS-B) antibodies. The left pleural effusion also showed an increase in anti-SS-A/SS-B antibodies. Soon after the initiation of corticosteroid administration, the left chest pain, malaise, and pleural effusion disappeared. We report a rare case of Sjögren's syndrome with left pleural effusion associated with suspected IgG4-related pleuritis.
