Necrotizing pneumonia due to Streptococcus pneumoniae following influenza infection is rare in an adult. A previously healthy 32-year-old woman was first diagnosed with type-A influenza and treated with peramivir in another clinic. After five days, she was diagnosed with pneumococcal pneumonia and admitted to another hospital. Chest X-ray revealed left pleural effusion, with pleural fluid glucose at 0 mg/dL and a pH of 7.0. She was treated solely with antibiotics for several days. After she was transferred to our hospital, a chest drainage tube was inserted and the left lung re-expanded. Chest computed tomography (CT) showed severe bilateral necrotizing pneumonia with destruction of lung parenchyma.
