Case Report

A case of anti-ARS antibody-positive interstitial pneumonia with eosinophilia in bronchoalveolar lavage fluid

ABSTRACT

A 66-year-old woman complained of dyspnea on exertion. Her chest CT showed ground-glass opacities on bilateral lower lobes, suggesting nonspecific interstitial pneumonia. She showed mechanic's hand and was positive for serum anti-aminoacyl tRNA synthetase (anti-ARS) antibody. Bronchoscopy revealed eosinophilia (30%) in the bronchoalveolar lavage fluid; thus, eosinophilic pneumonia needed to be ruled out. The bilateral lung shadows disappeared when prednisolone was commenced. Since anti-ARS antibody-positive interstitial pneumonia may show eosinophilia in bronchoalveolar lavage fluid, it is necessary to carefully distinguish it from eosinophilic pneumonia via image findings and physical examination.
