Interstitial pneumonia accompanied by reactivation of Epstein-Barr virus after administration of saireito

A 79-year-old woman was medicated with a herbal medicine, saireito, for dizziness. After eleven days' administration of this herbal medicine, she had fever and general fatigue, and was found to have interstitial pneumonia and liver dysfunction. Her chest computed tomography showed bilateral diffuse ground glass opacification. Bronchoscopy was performed and revealed elevation of atypical lymphocytes in bronchoalveolar lavage fluid (BALF). Epstein-Barr virus (EBV) DNA and BZLF-1 and BRLF-1 genes were detected from her BALF. We suspected that her interstitial pneumonia and liver dysfunction were drug-induced disorders due to saireito, accompanied by EBV reactivation. Although the mechanism of reactivation of EBV has not yet been elucidated, she recovered on withdrawal of the herbal medicine, and the administration of steroid pulse therapy and mechanical ventilation.
