A 77-year-old man with prostate cancer presented to our hospital with acute respiratory failure. Chest computed tomography showed mild ground glass opacity. The bronchoalveolar lavage fluid revealed diffuse alveolar hemorrhage. Histological findings of transbronchial lung biopsy showed an intraluminal tumor embolus. We diagnosed pulmonary tumor thrombotic microangiopathy (PTTM) induced by prostate cancer. Clinicians should recognize that alveolar hemorrhage can occur in cases of PTTM.
