A case of Paragonimus westermani infection in a Chinese immigrant

A 26-year-old Chinese man was referred to our hospital having had cough and hemoptysis for two months. He had a history of right pneumothorax and bilateral pleural effusion two years prior to presentation. A chest computed tomography (CT) scan revealed multiple pulmonary nodules with cavities mimicking pulmonary tuberculosis. Paragonimus westermani infection was diagnosed from identification of P. westermani ova in bronchial lavage fluid. Detailed history-taking revealed the ingestion of raw freshwater crabs before his right pneumothorax episode. He was treated with praziquantel, with rapid resolution of symptoms and nodules on chest CT.
Since immigrant patients to Japan are increasing, it is important to pay attention to infectious disease resulting from different eating habits.
