Case Report

A case of acute exacerbation of sarcoidosis with multiple organ involvement

ABSTRACT

A 37-year-old male visited our hospital because of blurred vision. His chest X-ray showed bilateral hilar lymphadenopathy. A few days later, he suddenly experienced severe dyspnea, and was initially hospitalized in the ICU, intubated, and mechanically ventilated because of hypoxemia. He rapidly recovered on treatment with steroid pulse therapy. Pathological examinations of skin and inguinal lymph node showed non-caseating epithelioid cell granulomatous lesions, and established the diagnosis of sarcoidosis. Upon further evaluation, multiple organ involvement was revealed (thorax, heart, eye, extrathoracic lymph nodes, skin, and nervous system). In cases of sarcoidosis involving multiple organs, the possibility of an acute exacerbation should be borne in mind.
