Case Report

A case of transformation of lung adenocarcinoma to squamous cell carcinoma after EGFR-TKI treatment

ABSTRACT

A 70-year-old woman was diagnosed with stage IV (cT1aN0M1b) lung adenocarcinoma exhibiting epidermal growth factor receptor (EGFR) mutation (exon 21 L858R) and brain metastases. Gefitinib was initiated, with a partial response. Twenty-five months later, she received chemotherapeutic treatment (pemetrexed) for disease progression. After seven cycles of pemetrexed, the disease had advanced further.
In order to determine the next stage of treatment, we performed a repeat biopsy at the initial biopsy site, which revealed that the cancer had undergone histologic transformation from an adenocarcinoma to squamous cell carcinoma. Here, we report a case of histologic transformation of lung adenocarcinoma to squamous cell carcinoma along with a review of the relevant literature.
