A case of alveolar hemorrhage by inhalation of metal fumes while arc welding

We report the case of a 20-year-old man who was a technical high school student with a history of metal allergy. He was admitted for pneumonia after inhalation of metal fumes during arc welding. On admission, he suffered from excessive hemoptysis, and a CT scan of the chest showed diffuse centrilobular nodules and ground-glass opacities in both lung fields. A bronchoalveolar lavage fluid was bloody and a cytodiagnosis of sputum showed hemosiderin-laden macrophages. The alveolar hemorrhage by inhalation of metal fumes was diagnosed on the basis of these findings and clinical course. To the best of our knowledge, this is a rare report and made us understand serious alveolar hemorrhage by inhalation of metal fumes while arc welding.
