A case of severe community-acquired pneumonia resulting from Legionella pneumophila serogroup 9

A 59-year-old man with diabetes and who was a heavy drinker was admitted because of fever and diminished consciousness. Chest radiography and computed tomography showed left-upper dense consolidation. Laboratory data upon admission revealed an increase of C-reactive protein level, liver enzymes, creatine kinase, and an electrolyte abnormality. Although Legionella urinary antigen testing was negative, based on these findings we suspected Legionella pneumonia. We treated him with intravenous azithromycin and levofloxacin. On hospital day 3, he was intubated because of respiratory failure. Culture of tracheal secretions yielded Legionella pneumophila serogroup 9, and we diagnosed him with community-acquired pneumonia resulting from Legionella pneumophila serogroup 9.
