Case Report

A case of IgG4-related disease with pleuritis

ABSTRACT

A 70-year-old man was admitted to our hospital because of having colon cancer. His FDG-PET-CT showed mediastinal lymphadenopathy and retroperitoneal fibrosis, he was suspected of IgG4-related disease. His FDG-PET-CT at first examination showed slight pleural effusion, and his chest CT two months later showed an increase of pleural effusion. His pleural effusion was exudative and showed an increased number of lymphocytes and a high level of adenosine deaminase. Immunological tests showed elevated IgG4 levels in his serum and pleural effusion. Histological examination of a pleural biopsy showed infiltration of numerous IgG4-positive plasma cells, and we diagnosed him as having IgG4-related disease with pleuritis. It is important to consider IgG4-related disease as one of differential diagnoses when we encounter patients with pleural effusion.
