Case Report

A case of hepatopulmonary syndrome revealed by complicating with acute exacerbation of interstitial pneumonia

ABSTRACT

A 76-year-old man was admitted to our hospital with dyspnea on exertion and hypoxemia. His chest CT showed bilateral diffuse ground-glass opacity with honeycomb and pulmonary vascular dilation in the lower lung. We diagnosed him as having acute exacerbation of interstitial pneumonia and treated with the administration of corticosteroid assisted by noninvasive positive pressure ventilation. His respiratory condition and chest CT findings had improved, but he had dyspnea and hypoxemia in a sitting position and was diagnosed as platypnea-orthodeoxia syndrome. He was then found to be complicated with alcohol-induced hepatic fibrosis, and we therefore diagnosed as hepatopulmonary syndrome (HPS) comorbid with interstitial pneumonia. We described a case of HPS revealed by complicating with acute exacerbation of interstitial pneumonia.
