ABSTRACT

The patient was a 71-year-old woman who had suffered Sjögren's syndrome. Five months ago, after a diagnosis of pneumonia, she was prohibited from taking all supplementary foods. She complained of cough and fever after readministration of a supplementary food containing reishi (Ganoderma lucidum). Chest CT showed diffuse ground-glass opacities in both lung fields. Lymphocytes were increased in bronchoalveolar lavage fluid. A transbronchial lung biopsy specimen showed pulmonary alveolitis. Therefore we diagnosed drug-induced pneumonia, not ruling out underlying collagen vascular disease associated, interstitial pneumonia (CVD-IP). In patients with connective tissue diseases, it is important to interview and consider a differential diagnosis that includes the possibility of drug-induced pneumonia.
