A 59-year-old woman with an elevation of lactate dehydrogenase was referred to our hospital. Chest CT revealed diffuse ground-glass opacities in both lungs with weak fluorodeoxyglucose uptake on positron emission tomography-CT (PET-CT). Interstitial pneumonia and intravascular lymphoma were suggested. A transbronchial lung biopsy was conducted but did not reach a definitive diagnosis. Biopsy specimens obtained by video-assisted thoracoscopic surgery contained a small amount of tumor cells, mostly in the vessels, and the diagnosis of intravascular large B-cell lymphoma (IVLBCL) was confirmed. Chest CT after the lung biopsy revealed a marked improvement of the diffuse ground-glass opacities, which was considered as a spontaneous remission of IVLBCL.
