Case Report

A case of IgG4-related disease with chronic cough and yearly progressive airflow obstruction

ABSTRACT

A 76-year-old man with diabetes mellitus and Sjögren's syndrome suffered from refractory cough for years. Annual pulmonary-function tests showed rapidly progressive airflow obstruction. Chest CT scan demonstrated marked thickening of the bronchial wall. IgG4-related disease was suspected because of his elevated serum IgG4 level, bronchial mucosa with infiltration by lymphocytes and IgG4-positive cells, and past history that were consistent with autoimmune pancreatitis and Mikulicz disease. His cough, airflow obstruction, and bronchial wall thickening were subsided by an administration of oral corticosteroid and bronchodilators. When encountering patients with refractory cough and remarkable thickening of the bronchial wall, consideration should be given to involvement with IgG4-related disease.
