A case of IgG4-related lung disease presenting with wandering shadows

A 69-year-old man was admitted to our hospital because of hypoxemia with chest-wandering infiltrates, which was unresponsive to treatment with antibiotics. Pathological findings obtained by video-assisted thoracoscopic lung biopsy revealed a nonspecific interstitial pneumonia pattern, including an abundant immunoglobulin (IgG) 4-positive plasma cell infiltration. Furthermore, his serum IgG and IgG4 levels markedly elevated. Therefore the patient was diagnosed to have IgG4-related lung disease, and he had been successfully treated with corticosteroids. We herein report a rare case of IgG4-related lung disease presenting with wandering shadows.
