A 63-year-old man with lung adenocarcinoma underwent a right upper lobectomy after gamma-knife radiosurgery for a single brain metastasis. He complained of hemosputum, and a bronchoscopic examination demonstrated polypoid nodules in the trachea and in the right and left main bronchi. A biopsy specimen demonstrated endotracheal/endobronchial metastases from lung adenocarcinoma. Thoracic radiation therapy and chemotherapy with gefitinib (Iressa) were simultaneously undergone to avoid death by hemoptysis, and the tumors had then almost disappeared. This patient was considered to be a rare case with endotracheal/endobronchial metastases of primary lung cancer.
