A 71-year-old man was referred to our hospital because of persistent right pleuritic chest pain. A computed tomography (CT) showed an anterior mediastinal tumor with right pleural dissemination and multiple pulmonary metastasis. Basaloid carcinoma was diagnosed by video-assisted thoracic surgery biopsy of the right pleural lesions. The thymus lesion was considered to be primary, according to the findings of positron emission tomography and CT. Five different regimens of combined cytotoxic agents based on lung cancer chemotherapy were given, but none were effective. However, the patient has been alive without serious complications two years and seven months since the disease was diagnosed. Thymic basaloid carcinoma is an extremely rare disease, and only 40 cases have been reported.
