A case of lung adenocarcinoma that transformed to small cell carcinoma

A 59-year-old woman, who had been diagnosed with lung adenocarcinoma T3N2M1b Stage IV with EGFR mutation (Exon 19 E746-A750del) and undergone chemotherapies (cisplatin+pemetrexed→erlotinib), was admitted to our hospital with a complaint of abdominal distention. The primary lesion was well controlled; however, the metastases to abdominal lymph nodes were developed. Lymph-node biopsies showed positive staining for both TTF-1 and synaptophysin, suggesting a transformation from adenocarcinoma to small cell lung carcinoma. Chemotherapeutic agents for small cell lung carcinoma tended to decrease the size of lymph nodes. Here we report a transformation case of lung adenocarcinoma to small cell carcinoma, with a review of relevant literatures.
