ABSTRACT

Thymoma is accompanied by a variety of paraneoplastic syndromes, such as MG. However, polymyositis (PM) is rare. Here we have experienced four cases of invasive thymoma accompanied with PM, presenting respiratory dysfunction. Among those, two have received noninvasive positive pressure ventilation, and one has required mechanical ventilation. Because respiratory failure frequently appeared in PM associated with invasive thymoma, we need to pay attention to the respiratory complication of PM. Magnetic resonance imaging (MRI) is good modality to assess muscle inflammation, even in impaired respiratory muscle, which is difficult to be performed in muscle biopsy.
