Case Report

A case of pulmonary emphysema accompanied by remarkable pulmonary arterial hypertension

ABSTRACT

Slight pulmonary emphysema and localized interstitial pneumonia were observed on chest CT scans. The respiratory function test revealed only decreased DLco. The cardiac catheterization revealed advanced pulmonary hypertension (PH) with pulmonary arterial pressure being 77/35 (mean: 51) mmHg. Because pulmonary thromboembolism, collagen vascular diseases, and pulmonary arterial hypertension caused by other diseases were excluded, we thought that this case had out-of-proportion PH associated with mild emphysema. The pulmonary arterial pressure estimated by a previous report, which uses the cross-sectional area of the small pulmonary vessels on a CT scan, approximated a measured value, thus suggesting the predictability of PH in emphysema from CT images.
