Case Report

A case of miliary tuberculosis with acute respiratory distress syndrome and recurrent bilateral pneumothorax

ABSTRACT

A 24-year-old man from China was admitted to the hospital because of acute respiratory failure. He had a three-month history of productive cough and progressive dyspnea. His CT scan of the chest on admission demonstrated diffuse ground-glass opacities, but miliary shadows were not detected. Smears of acid-fast bacteria stain were positive in his sputum and urine, and polymerase chain reaction tests for Mycobacterium tuberculosis were also positive. He was given a diagnosis of miliary tuberculosis complicated with acute respiratory distress syndrome. He required a mechanical ventilation in the intensive care unit and was treated with antituberculous chemotherapy, corticosteroid, sivelestat sodium hydrate, and so on. On the 6th day after admission, pneumothorax occurred on the right side, and on the 42nd day it occurred on the left. Tube drainage of both thoracic cavities were done. About four months later, these chest tubes were removed. After that, pneumothrax recurred once in the left side during hospitalization, and he required second drainage of his left thoracic cavity. We reported this case here because recurrent bilateral pneumothorax as a complication of miliary tuberculosis is rare.
