Case Report

A case of pulmonary infection resulting from Mycobacterium shimoidei

ABSTRACT

We report a rare case of pulmonary nontuberculous mycobacteriosis resulting from Mycobacterium shimoidei. A 77-year-old male was presented with cough. Chest radiography and CT showed a thick-walled cavity surrounded by small nodules in the right upper lobe. Sputum smears for acid-fast bacilli were positive. But the isolates were not identified by a polymerase chain method for mycobacterium tuberculosis or mycobacterium avium complex. Mycobacterium species were cultured in four weeks. Mycobacterium shimoidei was identified by the dnaj method. The combined therapy with rifampicin, ethambutol, and new macrolides improved his symptoms and abnormal chest shadows. Though there is no recommended therapeutic regimen for this disease, combined therapy including these drugs may have a beneficial effect on pulmonary infection because of Mycobacterium shimoidei.
