Obstructive pneumonia improved by biphasic cuirass ventilation: A case report

A 70-year-old man, who had needed full assistance in his daily life because of spinocerebellar degeneration, had a 2-week history of productive cough and fever before admission and was diagnosed as having left middle-lower lobe pneumonia. His pneumonia did not improve despite antibiotic therapy, and respiratory failure because of it as well as the inflammatory reaction progressed. It was thought that he had obstructive pneumonia with atelectasis based on sputum in the peripheral airway. On hospital day 34, he was put on biphasic cuirass ventilation (BCV). After the start of BCV, the pneumonia improved gradually, with no recurrence after he was weaned from BCV. In the present case, BCV was effective for the treatment of obstructive pneumonia in a patient who had difficulty in expectorating sputum.
