Case Report

A case of pulmonary inflammatory pseudotumor suspected of being a prodromal finding of IgG4-related disease

ABSTRACT

A 73-year-old man was admitted to our hospital for evaluation of weight loss and polyclonal hypergammaglobulinemia with IgG4 elevation. Computed tomography (CT) revealed marked right pleural thickening and hydronephrosis with retroperitoneal fibrosis. CT-guided needle biopsy of the pleura showed marked lymphoplasmacytic infiltration with IgG4-positive plasma cells; these findings met the criteria for IgG4-related disease. Tissue preparations from a pulmonary inflammatory pseudotumor resected 3 years previously had shown slight infiltration by IgG4-positive plasma cells. We hypothesized that in this patient, the pulmonary inflammatory pseudotumor was a prodromal finding of incipient IgG4-related disease.
