Case Report

A case of sarcoidosis that was differentially diagnosed from idiopathic interstitial pneumonia by video-assisted thoracoscopic surgery biopsy

ABSTRACT

A 59-year-old woman presented to a local hospital with cough, wheezes, and dyspnea on exertion. Her chest X-ray film showed reticulonodular shadows in both lower lung fields. Because a diagnosis could not be made using transbronchial lung biopsy, she was referred to our hospital for further examination. The shadows on her chest X-ray film had worsened, and she had obstructive impairment on a pulmonary function test. Biopsy specimens obtained by video-assisted thoracoscopic surgery showed noncaseating epithelioid cell granulomas consistent with sarcoidosis. Steroid therapy was started, and her dyspnea and hypoxemia on exertion improved. When interstitial pneumonia is predominantly found in the lower lung fields, sarcoidosis should be considered in the differential diagnoses.
