This case involved a 24-year-old man who developed pneumonia at 3 years of age and underwent surgery for pneumothorax aged 20. He was diagnosed with Swyer–James syndrome at 24 years of age. Chest computed tomography at that time showed multiple lung abscesses in saccular bronchiectasis. Antibiotic treatment resulted in temporary improvement, but multiple relapses adversely impacted his daily life. He was determined to undergo surgical treatment, so we performed resection of the left S1+2 segment and the part of the left S6 segment that showed hyperlucency on chest computed tomography. No subsequent relapses of lower respiratory tract infection were seen, and the patient exhibited a stable clinical course.
