A case of drug-induced lung disease following readministration of Sai-rei-to in a patients with a history of Durg Rash

A 67-year-old man with Ménière‘s disease initially received Sai-rei-to from an otolaryngologist. However, he developed a rash from the medicine and discontinued taking it. Six years later, he took it again, resulting in fever and respiratory distress. Chest-X-ray and computed tomography scans showed diffuse ground-glass opacities in the lungs, and he was diagnosed with drug-induced lung disease associated with Sai-rei-to, which improved with steroid treatment. Herbal medicines containing Scutellariae radix (a component of Sai-rei-to) have been frequently reported to cause drug-induced lung injury. There are also many reports of allergic side effects, such as drug rashes, associated with herbal medicines containing Scutellariae radix. Therefore, it was considered that Scutellariae radix was responsible for the drug rash and drug-induced lung disease in this patient.
