A case of idiopathic pulmonary fibrosis with recurrent pneumothorax during nintedanib therapy

An 80-year-old man with idiopathic pulmonary fibrosis underwent a chest computed tomography (CT) scan, which revealed progressive fibrosis and honeycombing at the lung bases. He started nintedanib and developed a pneumothorax 8 months later. The patient stopped taking nintedanib and rested, which quickly resolved his condition. He resumed nintedanib treatment and was admitted to the hospital 5 months later for another pneumothorax. We inserted chest tubes because he experienced a further pneumothorax and performed pleurodesis. However, these measures were ineffective. Therefore, a surgical repair was performed. Unfortunately, the patient developed postoperative pneumonia and died. Nintedanib initiation should be carefully considered in patients who are at high risk of developing a pneumothorax.
