A 71-year-old woman was diagnosed with Stage IVA primary lung adenocarcinoma. Despite treatment with gefitinib, the cancer spread to the right hilar lymph nodes, and chylous intrapulmonary and right pleural effusion appeared. Therefore, osimertinib was initiated as a second-line treatment. Although the tumor size reduced, the pleural effusion continued to increase. Because no improvement was observed with dietary treatment, lymphangiography was performed. Although it was difficult to identify the site of leakage, the pleural effusion decreased. Lymphangiography is potentially a therapeutic tool for the treatment of non-traumatic chylothorax.
