A case of MPO-ANCA-positive granulomatosis with polyangiitis with pleuritis as the only complication

An 82-year-old man presented with nosebleeds, exertional dyspnea, and dry cough, with a right pleural effusion noted on chest radiography. Because the pleural effusion was a lymphocyte-predominant exudative pleural effusion without malignant findings or high adenosine deaminase, all cultures were negative, and the blood serum MPO-ANCA test was positive, pleuritis caused by ANCA-associated vasculitis was suspected. He was diagnosed with granulomatosis with polyangiitis complicated by pleuritis because he met both the Japanese diagnostic criteria and the Watts classification criteria. He was treated with corticosteroids and achieved remission. Although pleuritis caused by ANCA-associated vasculitis is a difficult condition to recognize, we report it here because it is important for differentiation.
