A 77-year-old man presented with cough and dyspnea on exertion after the fourth dose of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccine. Computed tomography showed bilateral diffuse, centrilobular, granular shadows. Eosinophils were increased in peripheral blood and bronchoalveolar lavage fluid, and histopathology showed eosinophilic infiltration into the bronchiolar region. He was diagnosed with eosinophilic bronchiolitis and improved with systemic corticosteroid therapy. Eosinophilic disease should be noted after SARS-CoV-2 vaccination. This is the first report of eosinophilic bronchiolitis associated with the SARS-CoV-2 vaccination.
