A case of diffuse alveolar hemorrhage with positivity for anti-ARS antibody

A 70-year-old male presented with fever and dyspnea. Chest computed tomography revealed the presence of ground-glass opacification. He underwent bronchoalveolar lavage with tracheal intubation and mechanical ventilation. Hemosiderin phagocytic macrophages were observed in the bronchoalveolar lavage fluid. Based on the diagnosis of diffuse alveolar hemorrhage, steroid pulse therapy was commenced, resulting in decreased ground-glass opacification. Further analysis revealed positivity for the anti-aminoacyl tRNA synthetase (anti-ARS: anti-EJ) antibody. Cases of anti-ARS antibody syndrome complicated by diffuse alveolar hemorrhage are rare. Nevertheless, the presence of autoantibodies (anti-ARS antibody, and anti-nuclear and anti-neutrophil cytoplasmic antibodies) should be confirmed.
