A non-smoking, 20-year-old woman was referred to our hospital after computed tomography of her chest revealed right pneumothorax, multiple cysts, and nodules in the upper and middle lobes of both lung fields. She was a non-smoker; however, imaging findings strongly suggested pulmonary Langerhans cell histiocytosis (PLCH). Video-assisted thoracic surgery was performed for a definitive diagnosis, and the results confirmed the diagnosis of PLCH. This case study describes a rare case of PLCH in a non-smoking, young woman.
