A 59-year-old woman was referred to our hospital for a right pleural effusion with shortness of breath and cough. Contrast-enhanced computed tomography (CT) showed thickening of the right pleura and soft tissue lesions extending into the spinal canal and around the descending aorta. Video-assisted thoracoscopic pleural biopsy revealed B-cell acute lymphoblastic leukemia/lymphoma. Remission induction therapy with a combination of vincristine, daunorubicin, cyclophosphamide, L-asparaginase, and prednisolone was started, and the pleural effusion and pleural lesions rapidly disappeared. This is a rare case of adult B-cell lymphoblastic leukemia/lymphoma in which pleural effusion led to its detection.
