Involvement of oral antithrombotic drugs in bloody sputum and hemoptysis

Antithrombotic drugs are now widely used for many diseases, including various forms of ischemic heart disease. Although these drugs may provoke undesirable bleeding, the risk of bloody sputum and hemoptysis has yet to be estimated. The author investigated 97 patients with bloody sputum and hemoptysis and found that 30 (30.9%) of the cases were taking oral antithrombotic drugs. Among patients taking these drugs, the first of causal pulmonary diseases of bloody sputum and hemoptysis turned out to be chronic obstructive pulmonary disease, which itself rarely shows such symptoms. The most frequently used drug was aspirin. Because bloody sputum and hemoptysis may lead to a fatal outcome, antithrombotic drugs should be carefully administered, especially to patients who have pulmonary diseases accompanied by structure destruction.
