A case of hypereosinophilic syndrome with eosinophilic infiltration, pulmonary thromboembolism, and deep venous thrombosis

A 55-year-old man presented with high fever and dyspnea. A chest X-ray and a chest CT scan showed diffuse infiltration in both lungs, and laboratory findings revealed severe eosinophilia in both peripheral blood and bronchoalveolar lavage fluid (BALF). Further examinations led to a diagnosis of hypereosinophilic syndrome (HES) with disseminated intravascular coagulation (DIC). Corticosteroid therapy rapidly reduced eosinophilia and pulmonary infiltration, but the DIC stage remained, suggesting that thromboembolism was complicated. The postcontrast CT scan showed pulmonary thromboembolism and deep venous thrombosis, which were successfully treated with warfarin. Interestingly, on CT scans we could distinguish consolidations of pulmonary infarction from diffuse eosinophilic infiltration.
