A case of Waldenström macroglobulinemia with lung tumors, interstitial infiltrations, and hypercalcemia

An 83-year-old man was admitted to our hospital because of visual loss, photophobia, and dyspnea on exertion. Monoclonal gammopathy (IgM-λ type) was detected in the serum and lymphoplasmacytic cells proliferated in the bone marrow. Chest CT scan showed tumor shadows in the right middle and lower lobes and interstitial shadows in both lung fields. These shadows were diagnosed as pulmonary involvements of Waldenström macroglobulinemia because transbronchial lung biopsy specimen showed IgM-λ-positive lymphoplasmacytic cells infiltrated in both tumor and lung interstitium. This patient developed blood hyperviscosity and hypercalcemia refractory to therapy.
