A 74-year-old woman presented with dyspnea on exertion. Chest radiograph showed cardiac enlargement and bilateral pleural effusion. The patient had been receiving gefitinib for lung adenocarcinoma for 2 years. Progression resulting from lung cancer and primary effusion lymphoma (PEL) were suspected. Based on immunostaining of cell block sections from the pleural effusion, a diagnosis of diffuse large B-cell lymphoma was made. Although Southern blotting analysis was not performed, PEL was also suggested from the clinical course. We experienced a patient who developed malignant lymphoma during treatment for lung adenocarcinoma.
