A case of cryptococcal pyopneumothorax

A 76-year-old woman lost consciousness as a result of CO2 and was admitted to our hospital. A chest X-ray and computed tomography showed shadows and cavity lesions in the left lung and left pneumothorax with bilateral pleural effusion. We treated her with noninvasive positive pressure ventilation and chest drainage. Cryptococcus neoformans was cultured from the pleural effusion, and a pleural-fluid cryptococcus antigen test was positive simultaneously. Therefore we diagnosed pyopneumothorax caused by C. neoformans. She received an antifungal drug and was treated pleurodesis with OK-432, and she responded well to antifungal therapy and pleurodesis. We suggest that pleurodesis is useful in this case of inoperable cryptococcal pyopneumothorax.
